St. Joseph’s Villa offers employees optional health insurance coverage under United Healthcare with three different plans:

  • Surest A2500 Gold
  • Surest D6500 Silver
  • United Healthcare Choice HAS Bronze

Both the employee and the Villa contribute to these plans. Spouses and children are eligible for coverage.

Plan Summaries

Members have the option to choose a Primary Care Physician (PCP) to serve as their personal physician and help coordinate their health needs. 

No referrals are required to see participating specialists. Members may schedule appointments directly with any specialist within the UnitedHealthcare network.

Members may also receive care from any licensed physician, whether in or out of the UnitedHealthcare network. However, out-of-pocket costs are typically lower when services are received from in-network providers.

Dental Insurance

St. Joseph’s Villa offers dental insurance for employees and family members through UnitedHealthcare.

What’s Covered

  • Preventative Care – This covers routine cleanings for all covered individuals and fluoride treatments for those under the age of 16, often with no deductible.
  • Basic Services – This includes common dental procedures, such as fillings and emergency treatment for dental pain.
  • Major Services – This includes more extensive dental care, including root canals and retainers.
  • Extensive Provider Network – Access to a large network of dental providers in both private and retail settings.

Vision Insurance

St. Joseph’s Villa offers a vision plan for employees and family members through UnitedHealthcare.

What’s Covered

  • Eye Exams – Comprehensive eye exams include a review of your vision and eye health, helping to identify potential concerns early.
  • Frame Benefit – Receive an allowance toward the purchase of eyeglass frames when using a network provider.
  • Contact Lens Benefit – Coverage includes contact lenses, a fitting, and up to two follow-up visits. Select brands may be covered in full after copay.
  • Lens Upgrades – Save up to 40% on popular lens enhancements and upgrades.
  • Additional Pair of Glasses – Receive a 20% discount on additional pairs of prescription eyeglasses, including prescription sunglasses.

Life Insurance

Protecting your family’s financial future is an important part of your benefits package. Life insurance can help provide support and stability for your loved ones.

Employer Paid: Full-Time Salaried Employees

As a full-time salaried employee, you receive, at no cost to you, a Guaranteed Issue Amount of two (2) times your salary (to a maximum of $150,000) of life insurance through United Healthcare. This benefit becomes effective the first of the month following your date of hire.

You may choose any coverage offered up to the Guaranteed Issue Amount, without providing proof of good health, if you are a new hire and elect coverage within 31 days. If your active service ends due to disability, at age 60 or over, your life insurance coverage will continue while you are disabled.

Employee Paid: Full-Time Salaried and Hourly Employees

All full-time salaried and hourly employees of St. Joseph’s Villa are eligible for coverage. Your spouse (up to age 70) and child(ren) are also eligible as long as you apply for and are approved for coverage yourself.

Accidental Death & Disability (AD&D) Insurance

If you pass away or are seriously injured as a result of a covered accident or injury, you or your beneficiaries will receive a set amount to help pay for unexpected expenses, or help your loved ones pay for future expenses after you’re gone.

Available Coverage

Disability insurance, paid by St. Joseph’s Villa, covers a portion of your salary if you’re unable to work due to a covered disability.

Short-Term Disability Insurance

St. Joseph’s Villa provides short-term disability coverage for all full-time salaried employees, with eligibility beginning on the first day of the month following your date of hire. Benefits are subject to plan provisions, including a 30-day elimination period for the birth of a child, with a maximum weekly benefit of $1,000.

All full-time salaried employees are eligible for coverage the first of the month following date of hire.

St. Joseph’s Villa offers a 403(b) retirement savings plan through Vanguard to help employees prepare for their financial future. Employees may begin contributing through payroll deduction immediately upon hire.

Employees become eligible for the Villa’s matching contribution on the first day of the calendar quarter following the completion of one year of service and at least 1,000 hours worked. Once eligible, the Villa matches employee contributions up to 5% of eligible earnings. Employees are immediately 100% vested in all employer matching contributions.

Vanguard offers a variety of investment options to help employees build a retirement strategy that aligns with their individual goals. Contributions may be allocated among multiple funds or directed to a single investment option.

Investment selections may be updated at any time.

Legal Resources is a low-cost voluntary benefit that provides immediate and ongoing access to comprehensive legal coverage, helping you manage both anticipated and unexpected legal expenses.

Most legal services are covered at 100%, including attorney fees for:

  • Legal advice and consultation
  • Will preparation
  • Purchase, sale or refinance of primary residence
  • Traffic court representation
  • Uncontested divorce representation
  • Tenant-landlord disputes
  • Uncontested domestic adoption
  • Review of financial contracts or leases
  • District course representation in civil actions
  • Defense of a child in juvenile court for misdemeanor offenses

Members have access to a network of full-service law firms locally and nationally.

AFLAC provides cash benefits paid directly to the employee to help offset out-of-pocket medical expenses. We offer several voluntary insurance plans through AFLAC that may be added as a pre-tax deduction, including:

Employees have the opportunity to meet individually with an AFLAC representative to review plan options and determine the coverage that best meets their needs.
