Our Services

Community Stabilization

Community-Based Crisis Stabilization and Transitional Support

Our Community Stabilization program offer short-term, intensive mental health support for individuals experiencing an acute psychiatric crisis that puts them at risk of hospitalization. We aim to quickly stabilize mental health needs while keeping individuals within their community and close to their support networks.

We provide support:

  1. between an initial Mobile Crisis Response and entry in to an established follow-up service at the appropriate level of care;
  2. as a transitional step-down from a higher level of care if the next level of care is identified but not immediately available for access; or
  3. as a diversion to a higher level of care.

Our Services

  • Comprehensive assessments of mental and behavioral health
  • Short-term therapeutic interventions and skill-building
  • Support in identifying and activating natural support systems
  • Advocacy and networking to provide connections to additional community resources

How We Help

Qualified mental health professionals come meet the individual where they are, either at home or in a convenient community setting. We engage both the individual and their support systems to de-escalate and stabilize the crisis. Our primary goals are to prevent the escalation of mental health issues, ensure the safety of the individual and others, and deliver care in the least restrictive environment.

We use evidence-based interventions, including Cognitive Behavioral Therapy (CBT) and Applied Behavior Analysis (ABA), to help the individual develop skills to prevent future crises. Our approach is trauma-informed and strength-based, focusing on the individual’s resilience, problem-solving abilities, and capacity for post-traumatic growth.
